North Coast Health is conveniently located in Lakewood, Ohio, just 10 minutes from Downtown Cleveland. We provide health care services to the uninsured and underinsured, as well as to individuals covered by Medicaid, Medicare and other programs.
We are open five days per week AND on weekday evenings and Saturdays.
We offer:
Primary Health Care
Whether you need care for an acute need, such as an ear infection or strep throat, or care for a chronic condition such as diabetes or hypertension, we are here to help. We also work with our patients to help them stay well, so that they don’t develop chronic illnesses that they may be at risk for. Our clinical care team will take care of your immediate need and work with you to set goals and establish a care plan to help you stay as healthy as possible in the future.
Prescription Assistance
We work to provide free or low-cost medications to our uninsured and under-insured patients needing prescription medications. We work to enroll eligible individuals in pharmaceutical manufacturers’ patient assistance programs to help them get the medications they need to return to good health and stay well. In 2013, we dispensed $4.6 million in free prescription medication to our uninsured patients. We also have a discount program available for eligible patients through Discount Drug Mart for medications not available to through the pharmaceutical manufacturers’ patient assistance program.
Women’s Health Care
We offer well woman exams, including PAP and pelvic examinations, to enable optimal reproductive health. We make referrals to our hospital partners for mammograms and other tests and exams as needed.
Immunizations
We offer immunizations such as Pneumovax, Tdap and flu vaccines to patients according to age- and condition-specific immunization guidelines. We refer our patients to the Cuyahoga County Board of Health for immunizations that we are not able to offer on-site.
Referrals for Tests and Labs
We work with our local hospital partners to refer patients for needed tests and labs. Our patients are required to comply with the hospitals’ processes for applying for charitable care or they will be responsible for the costs of tests or labs ordered. Our clinical care team is committed to prompt follow-up on labs and test results for every patient.
Referrals for Specialty Medical Care
We connect our uninsured patients needing specialty care to our network of volunteer specialists or refer them to a specialist through Cuyahoga Health Access Partnership (CHAP).
Assistance with Coverage Enrollment
North Coast Health has trained assisters on site to help individuals enroll in Medicaid and in Marketplace plans. This assistance is offered both to current patients and to members of the community needing help with enrollment. In addition to offering enrollment assistance during open enrollment, we offer help year-round with Marketplace appeals, applications for exemptions, and enrollment during special enrollment periods following certain life events that involve a change in family status (for example, marriage or birth of a child) or loss of job-based health coverage.
