Patient-Centered Medical Home
North Coast Health Ministry has achieved recognition as a patient-centered medical home, level 3, by the National Committee for Quality Assurance, becoming the first free clinic in Ohio to attain this distinction. The Patient Centered Medical Home is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family.
Outstanding Improvement in the Care of People with Chronic Illness
North Coast Health, as a partner practice, earned Better Health gold star for their outstanding improvement from July 2012 through June 2015. North Coast Health was in the top 10% (either overall, or within an insurance group – Medicare, Commercial, Medicaid or Uninsured) in terms of their improvement over our last five reports on our key measures in diabetes, and high blood pressure.
GuideStar
North Coast Health attained Gold-level participant status with GuideStar, an organization dedicated to gathering and disseminating information about all IRS-registered non-profit organizations.
Good News Giving
Good News Giving is a special effort of Northeast Ohio Media Group Charities that provides help to health and human service organizations in the Northeast Ohio area by providing free advertising for 50 agencies that were selected by votes submitted by the people in Northeast Ohio.
Crain’s Health Care Heroes
Executive Director Lee E. Elmore and Medical Director Philip E. Tomsik, M.D., were recognized by Crain’s Cleveland Business as 2013 Health Care Heroes for their work in the Cleveland community.
